Documents to bring to your appointment
Important: You must bring your photo ID to the appointment in order to be seen.
To complete your application for assistance, please bring the following required documents to your scheduled appointment:
- Government Issued Photo ID for all adults in the household
- Social Security Cards for all household members (government issued documents showing the full 9-digit SSNs will be accepted)
- Late Notice for the rent, mortgage, and/or utility bill for which you are requesting assistance
- Rental or Lease Agreement (if expired, bring most current one)
- Proof of Income for the last 60 days from all sources for all household members
- Proof of Basic Expenses for the last 30 days
- Benefits Information such as food stamps letter or child support verification letter (if applicable)
- Proof of Crisis Situation - Verifiable documentation of a decrease in income or increase in basic expenses
- Bank Statements - Previous 60 days of bank/pay card transaction history
PLEASE NOTE: If you miss your appointment, you will need to call 407-836-6500 to be rescreened for a new appointment.
